Must enroll by the end of 28 weeks of pregnancy **for Jefferson County only
Be a first-time mother (no previous live births) **for Jefferson County only
Meets income requirements (eligible for Medicaid and/or WIC)
Lives in an area where Nurse Family Partnership is available
